Admission Health Requirements for Medical Students

Medical students have specific health clearance requirements that are different than the general UCI student population. These requirements are mandated by the UC Irvine School of Medicine. Please use the School of Medicine Immunization Form to document your immunizations, immunity and TB screening/testing. Follow the instructions on the form.

Submit all documentation directly to the UCI Student Health Center – Medical Records Department. SHC maintains all health clearance records for medical students and monitors/reports compliance on behalf of SOM.

All documents should be submitted to the Student Health Center either by mail, fax or in person:

You may email general inquiries regarding SOM health clearance requirements to SHC Immunizations. However, this email is not secure. DO NOT send Protected Health Information or Personally Identifiable Information (PHI/PII) to this email address.

If you need to send an inquiry or message to the Student Health Center that contains PHI/PII or sensitive information:

  • Logon to the secure Wellness, Health & Counseling Services Student Health Patient Portal.
  • Select "Messages" on the left sidebar of the main portal homepage.
  • Under "Please Select a Department for Secure Messaging", select the "Student Health Center" option.
  • Under "SHC Secure Message Options", select the "Front Office/Medical Records" option and click on "Continue" to compose and submit your secure message.
