COVID Bivalent Compliance was due January 6, 2023.
IMPORTANT NOTICE: UCI Student Health Center will hold COVID Bivalent Booster "Walk-In" Clinics on Wednesday, February 15, 2023 and Thursday, February 16, 2023 from 1:30 PM to 4:00 PM. The clinics will be held in the SHC I (building 5) conference room. No appointment necessary. The clinics are for students only and COVID vaccinations remain free to all students.

COVID Bivalent Booster Reporting Instructions
- Go to Medical Clearance > COVID-19 Vaccine > Update
- Upload the COVID bivalent booster immunization record
- Add Immunization Enter date COVID bivalent vaccine received and select one of the following options:
- “Moderna Bivalen (COVID19 Moderna Bivalent 18+)”
- “Pfizer Bivalent (COVID19 Pfizer Bivalent 12+)”

COVID-19 Bivalent Booster Vaccine Declination
You must either report you received the bivalent booster by entering your vaccination date and uploading your record (see instructions above) or complete the declination process by the compliance date.
Students should upload the Vaccine Declination Letter to the Student Health Patient Portal, MEDICAL CLEARANCES page prior to the deadline by following DIRECTIONS HERE. PLEASE DO NOT upload this letter under IMMUNIZATIONS, RELIGIOUS EXCEPTIONS, or any other section or you will continue to be NON-COMPLIANT.
Any Covered Individual who is not vaccinated must observe additional non-pharmaceutical interventions if required by the university Please follow current general campus recommendations, or requirements should they be applied, for masking and testing
COVID-19 Bivalent Declination Instructions
- Download and complete the COVID Vax Declination Form
- Go to Medical Clearance > COVID-19 Vaccine > Update
- Upload the completed form
- Add Immunization Enter date form completed and select "COVBIVDecline (COVID19 Bivalent Decline Form)"
