Disability Services

The UCI Student Health Center is committed to ensuring that all students with disabilities have equal access to care.

The UCI Disability Services Center assists students with disabilities, so they have equal access to all educational programs and activities on campus. The UCI Student Health Center offers a wide range of services to help students with disabilities achieve their academic endeavors while managing their important health and wellness needs, including support for appropriate approved accommodations.

Please “REGISTER” with the UCI Disability Services Center and complete the “DSC INTAKE FORM”. They will provide you with the appropriate documentation/forms to be submitted to your confidential file for review and support. Depending on both your disability and the accommodation requested, you may need your Primary Care Provider and/or Specialist to complete documentation.

  • If you have been treated by an off campus/community Primary Care Provider or Specialist for this disability, that provider should complete the DSC form.
  • If you have been treated by a UCI Student Health provider, speak with that provider about your request to get the process started.
  • If you are not currently connected with a Primary Care Provider, contact your Insurance Plan directly. If you have UC-SHIP, you may call this number for more information: 949.824.5301

You may need to have your DSC forms completed by a Mental Health provider (note: forms typically cannot be completed on a first visit. Please allow adequate time to establish care). 

  • If you have been treated by an off campus/community Mental Health provider, that provider should complete the DSC documentation.
  • If you are currently being treated by a UCI Mental Health provider, speak with that provider about your request to get the process started.
  • If you are not currently connected with a Mental Health provider, contact your Insurance Plan directly. If you have UC-SHIP, you may call this number for more information: 949.824.5301
